ABSTRACT

Scorpionism is a worldwide problem that has already made thousands of victims, and multi-disciplinary approaches for controlling their populations are to be more successful. Hens are often mentioned as tools for controlling scorpions; however, systematic/experimental behavioral studies are not available. Moreover, there is no systematic information on the effect of scorpion venoms on hens. Using the venomous yellow scorpion Tityus serrulatus, the present study aimed to clarify the following aspects: (1) voracity of hens, (2) how hens react when stung, (3) the effect of scorpion stings on hen behavior during attacks, and (4) hen survivorship after feeding on scorpions. Methods: We attracted hens with corn powder, offered them scorpions and then recorded the hen-scorpion interaction. To test the effects of the sting we manually removed the scorpion's telson. Results: We found that some hens ate up to six scorpions within minutes. By means of an ethogram and drawings, we showed that they exhibited several aversive behaviors when capturing scorpions. Removal of the scorpion telson stopped the aversive reactions, which was not observed in the control group. Finally, hens did not exhibit atypical behaviors after 1, 7 and 30 days and were all alive after 30 days. Conclusion: This is the first empirical and video recorded study providing evidence that hens are clearly affected by scorpion venom but do not die. Therefore, they may have potential to be used in biological control of these arthropods.(AU)

OBJETIVO: descrever as características das internações por causas externas envolvendo contato com animais em um hospital geral no interior da Bahia, no período de 2009 a 2011. MÉTODOS: estudo descritivo, com dados sobre internações no município de Jequié-BA. RESULTADOS: foram identificadas 246 internações por acidentes envolvendo contato com animais, sendo principalmente peçonhentos (83,3% por veneno de serpente e 6,5% por veneno de escorpião); predominaram vítimas do sexo masculino (66,7%), do grupo etário de 20 a 59 anos (50,4%) e residentes na zona rural (91,2%); a maioria das internações ocorreu no turno da noite (39,0%) e em dias úteis da semana (69,1%); a maior parte dos casos teve um tempo de internação de 1 a 3 dias (50,8%) e 97,6% deles evoluíram com alta hospitalar. CONCLUSÃO: ofidismo e escorpionismo foram os acidentes mais frequentes, acometendo predominantemente homens jovens residentes na zona rural.


OBJECTIVE: to describe the characteristics of hospitalizations due to external causes involving contact with animals in a general hospital in the interior of Bahia State (BA), Brazil, from 2009 to 2011. METHODS: this was a descriptive study using data on hospital admissions in Jequié-BA. RESULTS: there were 246 admissions owing to accidents involving contact with animals, especially venomous ones (83.3% snake venom, and 6.5% scorpion venom); most involved male victims (66.7%), the 20-59 year age group (50.4%), and people resident in rural areas (91.2%); most hospitalizations occurred during the night shift (39.0%) and on weekdays (69.1%); most cases had 1 to 3 day inpatient stays (50.8%), and 97.6% were subsequently discharged. CONCLUSION: snakebites and scorpion stings were the most frequent accidents and predominantly affected young men living in the rural area.

ABSTRACT

Abstract: Scorpion stings are currently the leading cause of venom-related injury to humans in Brazil and are a significant public health problem globally. Only scorpions of the Tityus genus are of medical importance in Brazil, and Tityus serrulatus is responsible for the most serious envenomations and deaths. The toxic effects of scorpion envenomation are due to a massive release of sympathetic and parasympathetic neurotransmitters; the severity is related to cardiac and hemodynamic changes, with cardiogenic shock and pulmonary edema contributing to the main causes of death. The pathophysiology of cardiac involvement has been discussed for decades and has been attributed to adrenergic discharge and a possible toxic effect of venom on the myocardium, while acute pulmonary edema may have a cardiogenic and/or non-cardiogenic origin. Currently, the clinical data point to catecholamine excess as the cause for reversible scorpion cardiomyopathy . These data include electrocardiographic changes, profiling of cardiac enzymes and troponin I, echocardiographic data with global or regional left ventricle dysfunction, and myocardial perfusion alterations compatible with spasm in the coronary microcirculation. Furthermore, recent data on cardiac magnetic resonance imaging findings, which are similar to those observed for stress-induced cardiomyopathy, have also been linked to catecholamine excess. The efficiency of antivenom serum treatment is controversial in the literature. Our experience in Brazil is that the management of patients with systemic manifestations of scorpion stings is based on three approaches, all of which are extremely important. These include symptomatic treatment, antivenom serum, and cardiorespiratory support.

ABSTRACT

Scorpions, mainly those belonging to the genus Tityus cause many deaths and injuries in Brazil, with tens of thousands of envenomations notified every year. However, injuries involving other scorpion species are scarcely registered. Among the sixteen species of the genus Rhopalurus, Thorell, 1876, described up to date, nine are found in this country, with only a confirmed case of human envenomation provoked by R. agamemnonKoch, 1839. The present case reports, for the first time, a case of scorpion sting in a human victim involving Rhopalurus amazonicus, endemic species of the west region of the Pará state, Amazon, Brazil. The symptoms of envenomation were local pain and paresthesia. This study contributes to develop the knowledge on venomous scorpions, particularly those that may cause envenomations in this region.

ABSTRACT

Background The present study compared two methods used successfully in a large-scale program for the collection of scorpion venoms, namely the milking of adult scorpions via manual and electrical stimulation. Results Our immunobiochemical characterizations clearly demonstrate that regularly applied electrical stimulation obtains scorpion venom more easily and, most importantly, in greater quantity. Qualitatively, the electrically collected venom showed lack of hemolymph contaminants such as hemocyanin. In contrast, manual obtainment of venom subjects scorpions to maximal trauma, leading to hemocyanin secretion. Our study highlighted the importance of reducing scorpion trauma during venom milking. Conclusions In conclusion, to produce high quality antivenom with specific antibodies, it is necessary to collect venom by the gentler electrical stimulation method.(AU)

ABSTRACT

Background: Previous works had shown that scorpion venom induced neurotransmitter elevation and an inflammatory response associated with various anatomo-pathological modifications. The most dangerous scorpions species in Algeria responsible for these effects are Androctonus australis hector (Aah) and Androctonus amoreuxi (Aam). Results: Comparison of the physiopathological effects induced by the two venoms showed differences in the kinetic of cytokine release and in lung injury. The lung edema was only observed in response to Aah venom and it was correlated with cell infiltration. In order to better understand the involved mechanism in inflammatory response, we used two antagonists, atropine (non-selective muscarinic antagonist) and propranolol (ß adrenergic antagonist), which lead to a decrease of cell infiltration but has no effect on edema forming. Conclusion: These results suggest another pathway in the development of lung injury following envenomation with Aam or Aah venom.(AU)

ABSTRACT

A patient with electrocardiographic abnormalities simulating early myocardial infarction after scorpion sting is reported. Pulmonary oedema and congestive heart failure accompanied these electrocardiographic changes. The aetiology of the cardiovascular manifestations in severe scorpion sting is related to the venom effects on the sympathetic nervous system and the adrenal secretion of catecholamines, as well as to the toxic effects of the venom on the myocardium.

ABSTRACT

Se determinó la prevalencia del envenenamiento escorpiónico en el Palmar-Edo. Bolívar durante el período comprendido entre enero 2005 y enero 2007. Hubo un total de 9 casos de escorpionismo (0,75 casos/1000 habitantes), afectando principalmente los miembros superiores de varones menores de 30 años, estudiantes, procedentes de zonas rurales. Los casos registrados ocurrieron durante el período lluvioso, entre los meses de abril y agosto. No se registraron casos mortales. Los hallazgos indican una casuística no despreciable de envenenamiento escorpiónico leve, sugiriendo la posible existencia de escorpiones poco tóxicos en la localidad estudiada


The prevalence of scorpion envenomation in El Palmar, State of Bolivar, was determined from January 2005 to January 2007. There were 9 cases of scorpion envenomation (0.75 cases/1000 inhabitants), affecting mainly the superior members of male students less than 30 years old that came from rural regions. Registered cases occurred in the rainy season between April and August. No fatalities were registered. Findings indicate a non-negligible prevalence of trivial scorpion envenomation, suggesting the possible existence of low toxicity scorpions in the locality under study

ABSTRACT

The most important health-threatening scorpions found in Turkey are; Androctonus crassicauda, Leiurus quinquestriatus, Mesobuthus gibbosus and M. eupeus species, all of which belong to the Buthidae family. The epidemiological and clinical findings of scorpion stings in Turkey were evaluated between the years 1995 and 2004 based on data recorded in the National Poison Information Center (NPIC). A total of 930 cases were recorded. The cases mostly occurred in the month of July. The gender distribution was 50.22 percent female and 45.48 percent male. It was shown that the 20-29 age group presented more scorpion stings. Most of the stings occurred in Central Anatolia and Marmara regions of Turkey. Patients at the hospital showed signs of localized (pain, hyperemia, edema and numbness) and systemic effects (hyperthermia, nausea and vomiting, tachycardia, shivering and lethargy) but no lethality was notified. According to records, 33 percent of the poisoned patients were treated with antivenin in healthcare facilities.


Os mais importantes escorpiões que comprometem a saúde na Turquia são: Androctonus crassicauda, Leiurus quinquestriatus, Mesobuthus gibbosus e M. eupeus, todos eles pertencentes à família Buthidae. Os achados clínicos e epidemiológicos da picada de escorpiões na Turquia foram avaliados no período entre os anos de 1995 a 2004 baseados em dados do Centro Nacional de Informações de Envenenamentos (NPIC). Foram registrados 930 casos que ocorreram principalmente no mês de julho. A distribuição por sexo foi 50,22 por cento femininos e 45,48 por cento masculinos. Demonstrou-se que o grupo etário de 20 a 29 anos apresentou maior número de picadas de escorpião. A maior parte das picadas ocorreu na Anatólia Central e na região de Marmara, na Turquia. Os pacientes hospitalizados mostraram sinais localizados (dor, hiperemia, edema e confusão mental) e sistêmicos (hipertermia, náusea e vômitos, taquicardia, arrepios de frio e letargia), mas não houve notificação de letalidade. De acordo com os prontuários 33 por cento dos pacientes foram tratados com antiveneno nas unidades de saúde.

Fundamentos: As crianças picadas por escorpião, pressintam ativação maciça do sistema nervoso simpática com vários graus de disfunção sistólica ventricular esquerda. Oobjetivo: Testar um protocolo de resgate em crianças com grave disfunção ventricular esquerda causada por picada de escorpião. Métodos: Quatro crianças após serem picadas por escorpião foram submetidas a: Encubação endotraqueal e suporte respiratório, eletrocardiograma, radiografia de tórax, ecocardiograma e determinação sérica da norepinefrina e troponina I. As análises foram repetidas após 12, 24 e 48 horas. As seguintes medicações intravenosas foram administradas: dobutamina 4-6 μg/kg/min; amiodarona 3 mg/kg durante duas horas, com dose de manutenção de 5 mg/kg/dia; e furosemida 0,5 mg/kg. Amiodarona, dobutamina e furosemida foram administradas durante as primeiras 48 horas. Bloqueadores beta-adrenérgicos e inibidores da enzima conversora da angiotensina foram administrados até 48 após a internação, uma vez que o estado clínico havia melhorado e a fração de ejeção ventricular esquerda encontrava-se acima de 0,35 por cento. Resultados: Na admissão, a dosagem da norepinefrina foi 1.727,50± 794,96 pg/ml, a de troponina I 24,53 ± 14,09 ng/ml e a fração de ejeção do ventrículo esquerdo foi 0,20 ± 0,056. Após 12 horas, os níveis séricos de norepinefrina e de troponina I diminuíram para a metade dos valores iniciais e a fração de ejeção aumentou para 0,32 ± 0,059. Durante as 24 e 48 horas subseqüentes, a fração de ejeção elevou-se para 0,46 ± 0,045 (p<0,01) e a norepinefrina e de troponina I diminuíram para 526,75 ± 273,73 (p< 0,02) e 2,20 ± 2,36 (p<0,02) respectivamente. Conclusão: É bem provável que a amiodarona, ao agir como neuromodulador, seja responsável pela redução rápida e progressiva dos níveis séricos de norepinefrina.


Background: Children with scorpion envenomation have massive sympathetic activation and variable degrees of left ventricular systolic dysfunction. Objective: To evaluate a rescue protocol for children with severe left ventricular dysfunction secondary to scorpion envenomation. Methods: Four children, after scorpion envenomation, were subjected to a rescue protocol for acute left ventricular dysfunction: Endotracheal intubation and respiratory assistance, electrocardiograms, chest x-Ray, echocardiograms and blood samples for norepinephrine and troponin I serum levels. Samples and echocardiograms were repeated at 12, 24 and 48 hours. Intravenous medications: Dobutamine: 4-6 μg/kg/min. Amiodarone: 3 mg/kg during a 2 hour period. Maintenance: 5 mg/kg/day. Furosemide: 0.5 mg/kg/dose. Diuretics were given when the systemic blood pressure was above percentile fifty. Amiodarone, Dobutamine and Furosemide were administered during the first 48 hours. Beta-adrenergic blockers and angiotensin converting enzyme were given, at 48 hours after admission, once the left ventricular Ejection fraction > 0.35 and the clinical status had improved. Results: On admission, norepinephrine was 1,727.50 ±794.96 pg/ml, troponin I 24.53 ± 14.09 ng/ml and left ventricular ejection fraction 0.20 ± 0.056. At twelve hours, norepinephrine and troponin I serum levels were down to half of the initial values and the ejection fraction increased to 0.32 ± 0.059. During the next 24 and 48 hours, the ejection fraction rose to 0.46 ± 0.045, (p<0.01) and norepinephrine and troponin diminished to 526.75 ± 273.73 (p < 0.02) and 2.20 ± 2.36 (p<0.02) respectively. Conclusion: Amiodarone, by acting as a neuromodulator, is very likely responsible for the early and progressive decrease of serum norepinephrine.


Fundamento: Los niños con picaduras de escorpión sufren activación masiva del sistema nervioso simpático con varios grados de disfunción sistólica ventricular izquierda. Objetivo: Probar un protocolo de rescate en niños con disfunción ventricular severa izquierda ocasionada por picadura de escorpión. Métodos: Cuatro niños tras un escorpión picarlas se sometieron a: incubación endotraqueal y soporte respiratorio, electrocardiograma, radiografía de tórax, ecocardiograma y determinación sérica de la norepinefrina y troponina I. Los análisis se repitieron tras 12, 24 y 48 horas. Las siguientes medicaciones intravenosas se administraron: dobutamina 4-6 mcg/kg/min; amiodarona 3 mg/kg durante dos horas, con dosis de mantenimiento de 5 mg/kg/día; y furosemida 0.5 mg/kg. Amiodarona, dobutamina y furosemida se administraron durante las primeras 48 horas. Bloqueante betaadrenergicos e inhibidores de la enzima convertidora de la angiotensina se administraron hasta 48 tras la internación, una vez que el estado clínico había mejorado y la fracción de eyección ventricular izquierda se hallaba superior a un 0,35 por ciento. Resultados: Al ingreso, la dosificación de la norepinefrina fue 1727,50± 794,96 pg/ml, la de troponina I 24,53 ± 14,09 ng/ml y la fracción de eyección del ventrículo izquierdo fue 0,20 ± 0,056. Tras 12 horas, los niveles séricos de norepinefrina y de troponina I disminuyeron para la mitad de los valores iniciales y la fracción de eyección aumentó para 0,32 ± 0,059. Durante las 24 y 48 horas subsiguientes, la fracción de eyección se elevó para 0,46 ± 0,045 (p<0,01) y la norepinefrina y de troponina I se redujeron para 526,75 ± 273,73 (p< 0,02) y 2,20 ± 2,36 (p<0,02) respectivamente. Conclusión: Es bien probable que la amiodarona, al actuar como neuromodulador, sea responsable de la reducción rápida y progresiva de los niveles séricos de norepinefrina.

ABSTRACT

The present study aimed to investigate the effects of Mesobuthus tamulus gangeticus Pocock (Buthidae) venom on albino mice (NIH strain). Whole venom was obtained by electrical stimulation and its toxicity was determined in albino mice by subcutaneous envenomation. The venom LD50 was 2.5 mg kg-1 of mouse body weight. Toxic effects on different biochemical and enzymatic parameters in blood serum and other tissues of albino mice were determined after experimental envenomation with sublethal doses of M. tamulus gangeticus venom. Increased levels of glucose, uric acid and cholesterol, as well as decreased serum total proteins, were observed at 2 and 4 hours after the envenomation. In the liver and muscles, glycogen content dropped after venom injection. Moreover, M. tamulus gangeticus venom elevated the enzymatic activity of acid phosphatase (ACP), lactic dehydrogenase (LDH) and alanine aminotransferase (ALT) in the serum of albino mice. In conclusion, M. tamulus gangeticus can be considered a lethal scorpion species.(AU)

ABSTRACT

OBJETIVOS: Descrever e avaliar os acidentes escorpiônicos notificados no estado de Minas Gerais, em crianças e adolescentes, no período de 2001 a 2005, e calcular incidência e mortalidade e identificar fatores relacionados à evolução para óbito. MÉTODOS: Estudo de 14.406 fichas de notificação obrigatória do escorpionismo em Minas Gerais, de pacientes de 0 a 19 anos. Após análise univariada, com fatores relacionados a mortalidade (p < 0,25), foi realizada regressão logística para análise multivariada. RESULTADOS: Houve aumento das notificações dos acidentes escorpiônicos ao longo dos anos. A faixa etária em que mais ocorreu acidente foi de 5 a 9 anos. Dor local esteve presente em 95 por cento dos casos. A maioria dos casos foi classificada como leve (60 por cento), com utilização do soro em 42 por cento dessa população. A letalidade média foi de 0,7 por cento. O município de maior ocorrência do acidente foi a capital Belo Horizonte. O atraso de cada 1 hora até o primeiro atendimento representou aumento de 9 por cento na razão de chance de evolução para óbito, que foi 46 vezes maior no grupo grave do que nos grupos moderado e leve. Para cada ano a menos na idade do paciente houve um aumento de 13 por cento na razão de chance de evolução para óbito. CONCLUSÕES: Houve aumento do número de notificações ao longo dos anos estudados. Foram identificados como fatores com maior razão de chance de evoluir para óbito: atraso para o primeiro atendimento, ser classificado como grave e menor idade.


OBJECTIVES: To describe and evaluate notified scorpion envenomation incidents in children and adolescents in the Brazilian state of Minas Gerais between 2001 and 2005, and to calculate the incidence and mortality and identify factors related to fatal outcomes. METHODS: Analysis of 14,406 mandatory notifications of scorpion envenomation involving patients aged 0 to 19 years in Minas Gerais. After performing a univariate analysis of factors related to mortality (p < 0.25), multivariate analysis was carried out using logistic regression. RESULTS: The number of notifications of accidents involving scorpions increased over the period. The age group that suffered most accidents was 5 to 9 years. Local pain was reported in 95 percent of cases. The majority of cases were classified as mild (60 percent) and antivenom was given to 42 percent of this population. The mean mortality rate was 0.7 percent. The state capital, Belo Horizonte, was the municipality with the greatest occurrence of incidents. Each hour's delay before receiving first medical care was associated with a 9 percent increase in the odds ratio for the likelihood of death, which was 46 times greater in the severe group than in the moderate and mild groups. A decrease of 1 year in patient age was associated with a 13 percent increase in the odds ratio for the likelihood of death. CONCLUSIONS: There was an increase in the number of notifications over the period studied. The following factors were identified as linked with a higher odds ratio for the likelihood of death: delayed first medical care, being classified as a severe case, and younger age of victim.

ABSTRACT

Scorpions are venomous arthropods of the Arachnida class and are considered relatives of spiders, ticks and mites. There is not any study about the biochemical effects of Androctonus crassicauda (Olivier, 1807) venom. Therefore, in the present study, we aimed at evaluating the toxicity of the venom from A. crassicauda, which is responsible for a number of deaths of infants, children and adults in tropical and subtropical countries. For this purpose, rats (n=35) were divided into seven groups of five animals each; venom solutions (250µg/kg) were subcutaneously injected into rats; blood samples were taken from each animal at various times; and serum biochemical parameters were measured (levels of total proteins, total bilirubin, albumin, globulin, urea, creatinine, uric acid, glucose, cholesterol, triglycerides, sodium, chlorine, potassium and calcium, and the activity of the enzymes alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, gamma-glutamyl transferase, lactate dehydrogenase). Serum levels of glucose, cholesterol, aspartate aminotransferase, alanine aminotransferase and uric acid increased in envenomed animals, compared to controls. There was a statistically positive correlation between Na+ and Cl- ions.(AU)

ABSTRACT

Los accidentes escorpiónicos que involucran al género Tityus son la causa más común de los envenenamientos ocurridos en el Estado Mérida. Para estudiar la ocurrencia de estos accidentes de acuerdo a los distritos sanitarios del Estado, se evaluaron el registro de morbilidad mensual EP115 y mortalidad EP113, y la memoria y cuenta de la Coordinación de Epidemiología del Estado Mérida; los años revisados comprenden desde 1994 hasta 2003. En los reportes, se detectó que la mayoría de los accidentes ocurrieron en los distritos sanitarios de Mérida, Tovar y El Vigía. Sin embargo, las muertes registradas (11) ocurrieron en niños con edades iguales o menores a los 9 años y en los distritos sanitarios de Tovar y El Vigía, lo que hace suponer que pudieron ser ocasionadas por la especie Tityus zulianus, que es la más abundante en la zona.

Scorpion envenomation is common in many countries; however, its effects on pregnancy are still unclear. In the present paper, we described the effects of scorpion envenomation on pregnant patients. A retrospective study was carried out considering the clinical and laboratory exams of patients admitted to the emergency room of Habib Bourguiba Hospital, Sfax, Tunisia, from 1990 to 2004. Variability of these clinical and laboratory profiles according to maternal age, gestational age and number of previous parities was also discussed. Among 167 scorpion-envenomed women, age ranged from 17 to 42 years, 7.18 percent were pregnant. These presented symptoms similar to those of non-pregnant women envenomed by scorpions. Two pregnant patients developed intense pelvic pain and one manifested vaginal bleeding. Although the studied parameters showed non-significant differences, we could conclude that scorpion envenomation may lead to abnormal uterine contraction probably causing preterm delivery. Maternal disturbances induced by scorpion envenomation may influence the fetus development. The effects were more severe in the second trimester of pregnancy.(AU)

ABSTRACT

BACKGROUND: Myocardial toxicity following a sting by the Indian red scorpion (Mesobuthus tamulus) is a life-threatening medical emergency. A perusal of the published literature suggests that this problem has seldom been studied systematically. METHODS: We retrospectively studied the clinical presentation and echocardiographic findings in 24 patients (mean [SD] age 23.2 [11.7] years; 19 males) with myocardial toxicity caused by the sting of an Indian red scorpion (Mesobuthus tamulus). They were treated with inotropic support and diuretics depending on the requirement. At admission, oral L-carnitine was administered in a dose of 1980 mg/day in three divided doses till the left ventricular (LV) function normalized. None of the patients received digitalis, prazosin, hydrocortisone or antivenin. RESULTS: Extreme anxiety and severe pain at the site of sting were present in all the patients. Hypotension (n = 19), pulmonary oedema (n= 15) and acute renal failure (n=8) were the other presenting features. Chest X-ray revealed cardiomegaly in 8 and pulmonary oedema in 13 patients. Serum creatinine phosphokinase levels were elevated more than two times the upper limit of normal (200 IU/L) in 22 patients (92%). The mean duration of hospitalization was 5 days (range 3- 11 days). L-carnitine treatment resulted in significant reduction in the LV diameter (mm) in diastole (47.6 [6.2] v. 42 [6.1], p < 0.01) and systole (42 [7.1] v. 28.2 [4], p<0.001); end-diastolic volume (ml) (108.7 [31.9] v. 81 [26.7], p <0.01) and end-systolic volume (ml) (81.3 [30.9] v. 31.1 [10.7], p < 0.001); and significant improvement in the stroke volume (ml) 27.8 [13.2] v. 61.7 [6.2], p<0.001) and ejection fraction (%) (25.5 [12.8] v. 61.2 [6.5], p<0.001). All the patients responded well to treatment and none died. CONCLUSION: Our initial observations suggest a potential benefit with additional oral L-carnitine treatment in patients with myocardial toxicity caused by scorpion sting presenting with hypotension and severe LV dysfunction. These findings merit further study.

BACKGROUND: 25-30% fatality due to acute pulmonary oedema in victims of Indian red scorpion (Mesobuthus tamulus) sting have been reported from Western Maharashtra, India. The advent of prazosin in recent years has revolutionized the management of severe scorpion sting cases. Majority of cases developed acute pulmonary oedema in 4-8 hours in a hospital setting irrespective of control of their arterial blood pressure with six hourly oral prazosin regimen, these cases recovered with extra dose of prazosin. We developed a standardised protocol for acute phase of treatment of these cases with the aim of preventing the development of pulmonary oedema. METHOD: We compared scorpion sting cases managed by non-protocol conventional (NPC) treatment and those by standardised protocol (SP) that included three hourly dose of oral prazosin. SP group included severe scorpion sting cases admitted to general hospital at Mahad in the year 1998 (Jan.-Dec.). While those admitted in the year 1997 (Jan.-Dec.) before the SP was implicated were the NPC group. FINDING: Characteristics on arrival of severe scorpion sting patients SP (n-17) and NPC (n-15) groups were similar that more case 6 (35%) from SP group had several hypertension on arrival. On arrival two cases from SP group and one from NPC group had pulmonary oedema. 16 (94.11%) patients from SP group recovered uneventfully, compared with 8 (53.33%) in NPC group (p-0.05). 0% Vs 5 (38.46%) developed acute pulmonary oedema (p < 0.0001) from SP and NPC group respectively, three (one had on arrival two patients during hospitalization) from NPC group had massive pulmonary oedema recovered with i.v. nitroprusside drip (SNP). While from SP group one had massive pulmonary oedema on arrival recovered with i.v. SNP, other one had pulmonary oedema recovered with oral prazosin. Cool extremities (vasoconstriction) persisted 11.5 (5-20) VS 18 (12-26) hours in SP and NPC group respectively. INTERPRETATION: Compared with NPC management; development of acute pulmonary oedema prevented by standardised protocol regimen at rural setting.

ABSTRACT

The possibility of producing neutralizing antibodies against the lethal effects of scorpion toxins was evaluated in the mouse model by immunization with an immunogen devoid of toxicity. A toxic fraction (5 mg) from the venom of the scorpion Tityus serrulatus was entrapped in sphingomyelin-cholesterol liposomes. The liposomes were treated for 1 h at 37 degrees Celsius with a 1 per cent (w/w) trypsin solution in 0.2 M sodium carbonate buffer, pH 8.3. This treatment led to a strong reduction in venom toxicity. Immunization was performed as follows: mice were injected sc with 20 mug of the liposome-entrapped toxic fraction on days 1 and 21 and a final injection (20 mug) was administered ip on day 36. After injection of the immunogen, all mice developed an IgG response which was shown to be specific for the toxic antigen. The antibodies were measured 10 days after the end of the immunization protocol. In an in vitro neutralization assay we observed that pre-incubation of a lethal dose of the toxic fraction with immune serum strongly reduced its toxicity. In vivo protection assays showed that mice with anti-toxin antibodies could resist the challenge with the toxic fraction, which killed, 30 min after injection, all non-immune control mice.
